Type of Organization: Church or Faith-Based
Denomination: Methodist
County: United States
State: Arkansas
Type the UNIQUE part of the organization: Type “Mountain Home” and select “Mountain Home First UMC”. The city will be automatically filled in and you may continue with the rest of the application.
You will see the option to start the training module when you finish your application.
Please print and fill out the Chid Maltreatment Registry Check. This form must be notarized. (Hard copies are available at the church office)
